Referral Information
HOW DO I REFER?

An appointment with a General Practitioner (GP) is usually the first step to gain a referral.
A GP can develop a Mental Health Care Plan with you to be able to access the medicare rebate.
People referred under Medicare will pay the full fee and receive a rebate for the service, there is an out of pocket expense.
The Mental Health Care Plan can be faxed to our friendly admin team on 03 4206 7738 or emailed at admin@mentalhealthcg.com.

There is also of course the option to pay the full fee without the need for a referral.
WHAT HAPPENS AFTER THE REFERRAL IS RECEIVED?
If a Mental Health Plan has been received via fax from the GP, we will call you to book an appointment. If you have not heard from us it is likely we haven't received your referral for some reason, please call us!
We will then email you a secure link to fill in your details, receive the client consent form and fill in the fee details.

If you prefer, you can call our practice first to book an appointment and then see your GP for the Mental Health Care Plan.

Alternatively you can book an appointment online and this will be confirmed, upon booking via email.
